REPAIR AUTHORIZATION FORM I hereby authorize Wright Collision Center, Inc. to make the specified repairs to my automobile. I hereby grant Wright Collision Center employees permission to operate my vehicle on city streets, highways, or elsewhere for the purposes of testing and/or inspection. An express mechanics line is hereby acknowledged on the vehicle to secure the amount of repairs hereto. Wright Collision Center will dispose of old parts removed from the vehicle unless otherwise instructed. Wright Collision Center will not be held responsible for loss or damage to the vehicle or article left in the vehicle in case of fire, theft, accident, or any other cause not do to negligence of Wright Collision Center or its employees. LIMITED POWER OF ATTORNEY I do hereby appoint Wright Collision Center to act as Power of Attorney in fact to accept on behalf any and all checks, drafts, or bills of exchange, and endorse all such checks, draft, or bills or exchange for deposit to Wright Collision Center account for credit on my account for repairs to my vehicle. I authorize any and all supplemental charges to be made payable to Wright Collision Center. VEHICLE RELEASE POLICY All repairs must be paid in full prior to vehicle's release unless prior arrangements have been made. All deductibles are payable to Wright Collision Center. I have read and understand Wright Collision Center release policy VEHICLE STORAGE FACILITY (VSF) & CONSENT VEHICLE STORAGE I understand that while my vehicle is being repaired, the vehicle may be stored in Wright Collision Center's vehicle storage facility. If stored in the VSF, your vehicle will be considered a body shop related vehicle and will not incur any VSF non consent storage fees. Your vehicle will not be considered a stored vehicle by the VSF. Your vehicle may incur body shop consent storage fees if 1. the vehicle is deemed a total loss by the insurance company or 2. the vehicle is not picked up within five days of the repair being completed. The body shop consent storage rate is $35.00 per day. CONSENT VEHICLE STORAGE AND ADMINISTRATIVE FEES Your vehicle may incur body shop consent storage fees if 1. the vehicle is deemed a total loss by the insurance company or 2. vehicle is not picked up within five days of the repair being completed or 3. the vehicle is dropped off for repairs but removed before repairs could be completed. If the vehicle is removed before repairs have been completed, the vehicle my incur charges related to the repair (i.e. work already done, towing, tear down, part charges, storage charges) and a $250.00 administrative fee. The body shop consent storage rate is $35.00 per day.
